• Copy of last year's tax return

• Itemized deductions
o Form 1098 (mortgage interest)
o Property taxes paid (if not on 1098)
o Car registration costs
o Charitable Donations
o Medical expenses (exceeding 7.5% of gross income)
• Other
o Form 1095 B (health insurance). If using Covered California, make sure your have your 1095-A as well as your for 3895
o Categorized and totaled list of expenses if independent contractor
o Contributions to IRA
o Name, SSN, DOB of new dependents
o HSA forms
o Form 1098-E (student loan interest)
o Form 1098-T (tuition for higher education)
o Education expenses (books, supplies, etc)
o Childcare expenses (summarized by provider and amount)
o Gambling losses
o Energy efficient home improvements
o Purchase of electric vehicle
